I have done two '04 Rangers with the exact same problem on the same bank in the last six months. Exhaust valve seats wearing way back into the head. Both came in for a miss. About 90 psi compression and failed leakdown tests. Lots of air coming from tailpipe during leakdown test.
Just wanted to throw this out there for reference.

Sounds like my '99 4.0 Ranger. Right head, rear cyclinder. Happens every 100,000 miles. About due for the 3 set of heads.
