Exporting COMPLETE Customer Contact Data

I asked this before, but sadly, I still haven't found the solution I need -

Someone her once helped me to run a report that could be used to export a complete customer contact database. That is, one that could export to excel in relatively clean format, and include the following: Name, Address, City, State, Zip, Phone, & Email Address.

Somehow, I've lost it...I can't seem to be able to run that report again. Can someone give me some advice on this?
